  • There is never a charge of any kind for an Angel Flight.
  • Your flight will be scheduled to accommodate your medical appointment.
  • Your pilot will pick you up at an airport near your home and bring you to your treatment facility.
  • Your flight is just for you, and will typically be in a small private plane.
  • This means that we can pick you up at an airport near your home, even if you live in a rural area.
  • After your appointment, another pilot will pick you up and bring you home.
  • You can bring companions to travel with you.
  • You can travel with Angel Flight on as many trips as you need.

Your Privacy

The privacy and security of our passengers’ personal information is very important to us. We will share passengers’ personal information only when necessary for the coordination and safety of the flights. We share private information with our volunteers and some third parties such as airlines and ground transportation providers as is required for the logistics and safety of the transportation. When a medical release is required as per our policies, some personal information will be shared as a part of this process. By submitting this request, you acknowledge that you understand how your or your patient’s personal information will be shared and agree to such use.

1.  Is this your first flight or request with Angel Flight West?*

If you have flown or made a request with Angel Flight West in the past, please call the office at (888) 4-AN-ANGEL to schedule your next flight.

2.  Is your donated flight needed after 10/01/2026?*

Generally, we require 5 business days advance notice to find a volunteer pilot for your flight. In some cases, we can accommodate shorter term requests.
You don't need to know the exact date of your appointment in order to complete the flight request, however, we will need to have a firm appointment before we can start looking for volunteer pilots for your flight. Please be aware that we generally require 5 business days advance, but we can sometimes accommodate shorter term requests.

3.  Do you have, or can you get a physician's medical release?* Help: What is a medical release and why do I need one?"

Before we can start looking for volunteer pilots to fly you, we will need to have a physician verify that the passenger is medically stable and able to fly in a small, unpressurized aircraft and that a passenger's condition will not affect the safety of the flight.

4.  Are you able to board a small aircraft on your own and sit up in a seat with a seat belt fastened around you? Are you able to travel without medical care during the flight?* Help: What is different about flying in a small plane?

If you require medical assistance during the flight, or cannot sit up for the duration of the flight, we recommend you contact a charitable air ambulance service. The Air Care Alliance may be able to help you find a suitable resource in your area.

5.  Are you traveling for a medical appointment, domestic violence relocation, or to be a caregiver for a child or a parent?*

Angel Flight West can assist you with travel to a scheduled medical appointment, for domestic violence relocation if you are currently in a shelter, or to be a caregiver for a direct family member (parent, child, spouse). We unfortunately cannot provide transportation for funerals or for permanent relocation.

6.  Do you have a financial need or a compelling reason why you can't use public transportation?*

Angel Flight West flights are flown by volunteer pilots who donate their time as well as the costs of the flight. Angel Flight West is a charitable service serving people in need. For this reason, we require that passengers have a financial need, or have a compelling reason why they cannot travel commercially.

7.  Where will this flight begin and end?

You don’t need to know the exact location of your appointment in order to complete the flight request, however, we will need to have a firm appointment before we can start looking for volunteer pilots for your flight. Please be aware that if the location falls outside our territory, we may need to refer you to another organization.
